The safety and wellbeing of our guests and retreat hosts, is our priority.

This form should be completed following any accident, injury, property damage or other incident occurring at Karmavida Retreats. All incidents should be reported promptly.

Information will be treated confidentially and used for safety, insurance and record-keeping purposes.
